East Croydon Medical Centre and Edridge Road Community Health Centre are very proud to announce that they have dedicated social prescribers who can help patients in a holistic and non-clinical manner.
The social prescribers at both ECMC and Edridge Road are Geeta Shah and James Moore.
Our lead social prescriber is Geeta Shah who has been working with the practice since August 2019. As part of her role she has been guiding patients in remaining active by joining local activities, supporting them with benefits, referring them to social services for care needs for equipment assessments and a lot more.
Prior to this Geeta had been supporting and signposting people with non-clinical needs for a number of years. She has been a Carer for her own elderly relatives and also worked as a befriender co-ordinator and a personal care independance co-ordinator with a large charitable organisation, enabling people to remain independant for as long as possible.
